Low Income Subsidy Form

The SDC Low Income Subsidy is provided for families who meet both of the following criteria:

  1. The participant cannot use any government-based disability funding

  2. The family annual income is less or equal to:

Single parent with a child: $18,166

Two parents with a child: $22,109

After completing this form, please email programs@socialdiversity.org with the following:

  1. “Notice of Assessment” for each adult in the family to confirm proof of income

  2. A picture of a government issued ID

Once your Low Income Subsidy has been submitted and the required forms have been received, an SDC Programs Coordinator will send you a confirmation email.
